Right at Home is seeking a Registered Nurse to join our Support at Home team as a Clinical Care Partner in Noosa, Gympie and Hinterland. You will provide clinical leadership, care coordination and oversight for clients receiving services through the Support at Home Program, managing your own caseload and developing care plans with budget awareness.

The role emphasises autonomous practice, person-centred care and collaboration with families, care workers and multidisciplinary teams.

Qualifications

• Current AHPRA Registered Nurse registration.
• Experience in community nursing, aged care, rehab or palliative care.
• Strong clinical assessment and care planning capability.
• Understanding of Support at Home classifications, funding and service delivery.
• Experience supervising or guiding care staff.
• Current First Aid & CPR.
• Driver’s licence and reliable vehicle.
• Police and screening clearance.
• Ability to work independently and travel to clients’ homes.

About the Role
