Job summary

Right at Home Central Queensland is hiring a Registered Nurse / Care Coordinator to support in-home care across Rockhampton and surrounding areas. You will ensure clients receive the right care at the right time, coordinating assessments, plans and service delivery with families and carers.

The role emphasises compassionate, patient-centred care, professional development, and collaboration with a dedicated local team. AHPRA registration and current First Aid/CPR are required.

Qualifications

• AHPRA registered nurse with current practicing certificate.
• Experience in aged care, community care, or disability support.
• Current First Aid and CPR certification and National Police Check.
• WWC check or willingness to obtain.

Responsibilities

• Coordinate and oversee in-home care services.
• Conduct care assessments, reviews, and service planning.
• Build strong relationships with clients, families, carers, and the care team.
• Monitor care delivery for quality, safety and compliance.
• Provide guidance and training to carers and staff.

About the Role

As our RN / Care Coordinator, you’ll play a key role in making sure our clients receive the right care, at the right time, in the comfort of their own homes.

Your day-to-day will include:

• Coordinating and overseeing in-home care services
• Conducting care assessments, reviews, and service planning
• Building strong relationships with clients, families, carers, and the wider care team
• Monitoring care delivery to ensure quality, safety, and compliance
• Supporting carers and staff with guidance, training, and regular updates
• Travelling throughout the Wide Bay and surrounding areas— with a company car provided for work-related travel

You’ll ideally bring:

• Previous experience in Aged Care, Community Care, or Disability Support
• Strong time management and organisational skills
• Excellent communication and interpersonal skills
• Knowledge ofcare standards, assessments, and service coordination
• A current driver's licenceand access to a reliable vehicle

• AHPRA Registered and practicing
• Drivers Licence
• Any other Aged Care / Health Care related certificates
• Proven experience in aged care, disability, or mental health within a community setting.
• Strong understanding of Aged Care and Disability Standards, as well as consumer-directed care principles.
• Current First Aid, CPR certification and National Police Check.
• Working With Children (WWC) check or willingness to obtain.
• A valid driver's license and access to a reliable car with comprehensive insurance.
• Knowledge of Home Care Packages (HCP) or Support at Home (SAH) (Desirable)
