Right at Home Noosa, Gympie, Hinterland is seeking a Registered Nurse to join as Clinical Care Partner, delivering autonomy and person‑centred home care. You will lead clinical supervision, coordinate services and guide care staff to support clients to live safely and independently at home.

You will manage a caseload, perform clinical assessments and develop care plans that align with client goals and budgets, while ensuring quality and safe outcomes across the Support at Home program.

Qualifications

  • Current AHPRA Registered Nurse registration.
  • Experience in community nursing, aged care, rehab or palliative care is preferred.
  • Strong clinical assessment and care planning capability.
  • Understanding of funding, service delivery and client classifications.

About the Role

As aClinical Care Partner, you will provide clinical leadership, care coordination and oversight for clients receiving services through theSupport at Home Program. You’ll manage your own caseload, conduct clinical assessments, develop care plans, support care staff, and ensure services align with each client’s classification, goals and available budget.

You’ll be a trusted clinical contact for clients, families, care workers and multidisciplinary partners ensuring care is appropriate, safe, coordinated and person‑centred.
