Job summary

Right at Home is seeking a Registered Nurse to join the Clinical Care Partner team, delivering autonomous, client-centred in-home care. You will lead clinical care, coordinate services, and supervise care staff while guiding clients to live safely and with dignity in their own homes.

You will manage your caseload, perform clinical assessments, and develop care plans aligned with client goals and funding. Travel to clients’ homes is expected, with strong clinical leadership support.

Qualifications

• Current AHPRA Registered Nurse registration.
• Experience in community nursing, aged care, rehab or palliative care.
• Strong clinical assessment and care planning capability.
• Understanding of Support at Home classifications, funding and service delivery.
• Experience supervising or guiding care staff.
• Current First Aid & CPR.
• Driver's licence and reliable vehicle.
• Police and screening clearance.
• Ability to work independently and travel to clients’ homes.

About the Role

As aClinical Care Partner, you will provide clinical leadership, care coordination and oversight for clients receiving services through theSupport at Home Program. You’ll manage your own caseload, conduct clinical assessments, develop care plans, support care staff, and ensure services align with each client’s classification, goals and available budget.

You’ll be a trusted clinical contact for clients, families, care workers and multidisciplinary partners ensuring care is appropriate, safe, coordinated and person‑centred.
