ForHealth Ballarat Medical Centre is seeking a CDM Nurse (RN or EEN) to work closely with GPs and the broader team. You will deliver chronic disease management and preventative care, supporting patients through care planning, health assessments, education and ongoing coaching.

Qualifications

• Current unrestricted AHPRA registration as RN or EN.
• Experience in general practice or primary healthcare.
• Experience in chronic disease management or preventative healthcare.
• Knowledge of Medicare-funded health assessments and care planning.

Responsibilities

• Support patients in setting SMART goals and action plans for chronic conditions.
• Provide ongoing nurse monitoring, education and health coaching.
• Coordinate care between GPs, specialists and allied health providers.
• Perform clinical services including ECGs, Spirometry, ABI testing and Urinalysis.
• Identify eligible patients for chronic disease management programs and preventative services.
• Maintain accurate clinical documentation and Medicare compliance.
• Build relationships with GPs and practice staff to promote preventative care.
• Educate nursing and administrative teams on chronic disease management processes.

About the Role

An exciting opportunity is available for a CDM Nurse (RN or EEN) to join our Ballarat medical centre.

Working closely with GPs and the wider healthcare team, you will play a key role in delivering high-quality chronic disease management and preventative healthcare services. About You

To succeed in this role, you will have:

• Current unrestricted AHPRA registration as a Registered Nurse or Enrolled Nurse.
• Previous nursing experience within general practice, primary healthcare or a similar clinical environment.
• Experience in Chronic Disease Management or a strong interest in preventative healthcare.
• Strong communication and patient engagement skills.
• Excellent organisational and time management abilities.
• A proactive, patient-centred approach to care.
• Knowledge of Medicare-funded health assessments, care planning and chronic disease management processes is highly regarded.
• Unrestricted Australian working rights.
