Job summary

integratedliving Australia Ltd in Ballarat invites an Enrolled Nurse to join our team on a permanent part-time basis (32 hrs/wk). You will travel to clients in Ballarat and surrounding areas, deliver direct care, health promotion, and Telehealth-enabled services to support older Australians living at home.

You’ll work independently with regular shifts, have access to a fleet car and salary packaging, and opportunities for professional development through our values-driven, supportive workplace.

Qualifications

• Understanding of community nursing and consumer-directed care.
• At least 2 years of experience in community, acute, hospital or aged care.
• Experience working independently or as part of a team.
• Strong organizational skills and attention to detail.
• Proficiency with electronic devices and client management systems.
• Current Registration with AHPRA, Police check, working with children check and CPR.
• Experience providing advanced clinical care including chronic disease, medication, palliative care, catheter management, stoma care, wound care, and diabetes management.

Responsibilities

• Provide health promotion and education to clients and their carers.
• Promote and deliver our Wellness for Independence programs, both in-person and virtually.
• Monitor and improve health outcomes through Telehealth.
• Direct care services across funding programs, including wound care, diabetes management, medication management, stoma care, and catheter care.
• Centered care: assess, develop, and evaluate nursing care plans with client goals and independence, with Allied Health support.

• Proficiency with electronic devices and client management systems (smartphones, tablets, laptops, email, etc.)
• Current Registration with AHPRA, Police check, working with children check and CPR
