Job summary

ForHealth Group in Ballarat is seeking a CDM Nurse (RN or EEN) to join our Ballarat medical centre. You will work with GPs and the healthcare team to deliver chronic disease management, prevention and education, helping patients achieve better long-term health outcomes.

You will conduct health assessments, support goal setting, provide coaching, and coordinate care across providers while ensuring Medicare compliance and accurate documentation.

Qualifications

• Current unrestricted AHPRA registration as a Registered Nurse or Enrolled Nurse.
• Experience in chronic disease management or preventative healthcare is preferred.
• Unrestricted Australian working rights.
• Vaccination history and immunity records available on request.

Responsibilities

• Prepare and coordinate GP Management Plans (GPMPs) and Team Care Arrangements (TCAs).
• Conduct health assessments including 75+ Health Assessments, Heart Health Checks, Diabetes Risk Assessments, ATSI Health Assessments, and Veteran/Refugee health assessments.
• Support patients in setting SMART health goals and action plans.
• Provide ongoing nurse monitoring, education and health coaching.
• Coordinate care between GPs, specialists and allied health providers.
• Perform clinical services including ECGs, Spirometry, ABI testing and Urinalysis.
• Maintain Medicare compliance and accurate documentation.
• Build relationships with GPs and practice staff to promote preventative care.

You will support patients living with chronic conditions through care planning, health assessments, education and ongoing health coaching, helping them achieve better long-term health outcomes.

Key Responsibilities

• Prepare and coordinate GP Management Plans (GPMPs) and Team Care Arrangements (TCAs).
• Conduct health assessments, including 75+ Health Assessments, Heart Health Checks, Diabetes Risk Assessments, ATSI Health Assessments, and Veteran, Refugee and Intellectual Disability Health Assessments.
• Support patients in setting SMART goals and developing action plans to manage chronic health conditions.
• Provide ongoing nurse monitoring, education and health coaching.
• Coordinate care between GPs, specialists and allied health providers to deliver holistic patient care.
• Perform clinical services including ECGs, Spirometry, ABI testing and Urinalysis.
• Identify eligible patients for chronic disease management programs and preventative healthcare services.
• Maintain accurate clinical documentation and Medicare compliance.
• Build strong relationships with GPs and practice staff to promote preventative healthcare and increase patient engagement.
• Support and educate nursing and administrative teams on chronic disease management processes.

About You

• Current unrestricted AHPRA registration as a Registered Nurse or Enrolled Nurse.
• Previous nursing experience within general practice, primary healthcare or a similar clinical environment.
• Experience in Chronic Disease Management or a strong interest in preventative healthcare.
• Strong communication and patient engagement skills.
• Excellent organisational and time management abilities.
• A proactive, patient-centred approach to care.
• Knowledge of Medicare-funded health assessments, care planning and chronic disease management processes is highly regarded.
• Unrestricted Australian working rights.
• The ability to provide evidence of vaccination history and immunity records.
