Job summary

Bolton Clarke Group is seeking a Funding Coordinator to lead AN-ACC funding processes at a Banora Point aged care home. You will collaborate with clinical teams to ensure accurate documentation, timely assessments and compliant funding claims, while coaching staff on best practices.

As an experienced RN, you will coordinate funding reviews, support admissions decisions, and drive continuous improvement within a supportive not-for-profit framework that emphasises quality of care and community

Qualifications

• Current registration with AHPRA as a Registered Nurse.
• Experience with AN-ACC funding and funding processes.
• Strong communication and coaching abilities.
• Knowledge of aged care legislation and funding frameworks.

Responsibilities

• Coordinate AN-ACC processes across the residential home.
• Review resident care needs and identify opportunities for funding optimisation.
• Ensure clinical documentation, assessments, care plans and progress notes accurately support funding claims.
• Partner with Registered Nurses, Clinical Managers, Allied Health Professionals and Medical Officers to gather supporting evidence.
• Monitor resident changes and facilitate reassessments where appropriate.
• Deliver education, coaching and support to clinical teams on AN-ACC requirements and best-practice documentation.
• Coordinate AN-ACC meetings and support assessment visits.
• Provide guidance on predicted funding outcomes for potential admissions.
• Work collaboratively with leadership teams to drive continuous improvement and compliance.

About the Role

As the Funding Coordinator, you will provide leadership and support in the delivery of the home's AN-ACC program, ensuring residents receive appropriate funding aligned to their care requirements. You will collaborate with clinical teams, allied health professionals, and medical practitioners to maintain high-quality documentation and assessment processes

To be successful, you will have

• Current registration with AHPRA as a Registered Nurse.
• Strong clinical assessment and documentation skills.
• Excellent communication, stakeholder engagement and coaching abilities.
• Strong organisational skills with the ability to manage competing priorities.
• Experience working as a Registered Nurse within aged care.
• Knowledge of AN-ACC funding and assessment processes.
• Experience in funding, quality, compliance or clinical coordination roles.
• Understanding of aged care legislation and funding frameworks.
