Job summary

Bolton Clarke Residential Aged Care Opportunities is seeking a Funding Coordinator in NSW to lead AN-ACC processes within a Winders Lodge residential home. You will align funding with resident care, collaborate with clinical teams and practitioners, and maintain high-quality documentation and assessments to support funding claims.

The role requires an AHPRA-registered nurse with strong assessment, documentation and coaching skills, plus knowledge of AN-ACC funding and aged care legislation.

Qualifications

• Current registration with AHPRA as a Registered Nurse.
• Strong clinical assessment and documentation skills.
• Excellent communication, stakeholder engagement and coaching abilities.
• Strong organisational skills with the ability to manage competing priorities.
• Experience working as a Registered Nurse within aged care.
• Knowledge of AN-ACC funding and assessment processes.
• Experience in funding, quality, compliance or clinical coordination roles.
• Understanding of aged care legislation and funding frameworks.

Responsibilities

• Coordinate and manage AN-ACC processes across the residential home.
• Review resident care needs and identify opportunities for funding optimisation.
• Ensure clinical documentation, assessments, care plans and progress notes accurately support funding claims.
• Partner with Registered Nurses, Clinical Managers, Allied Health Professionals and Medical Officers to gather supporting evidence.
• Monitor resident changes and facilitate reassessments where appropriate.
• Deliver education, coaching and support to clinical teams on AN-ACC requirements and best-practice documentation.
• Coordinate AN-ACC meetings and support assessment visits.
• Provide guidance on predicted funding outcomes for potential admissions.
• Work collaboratively with leadership teams to drive continuous improvement and compliance.

About the Role

As the Funding Coordinator, you will provide leadership and support in the delivery of the home's AN-ACC program, ensuring residents receive appropriate funding aligned to their care requirements. You will collaborate with clinical teams, allied health professionals, and medical practitioners to maintain high-quality documentation and assessment processes
