Job summary

Lives Lived Well is seeking an experienced AOD Counsellor / Case Manager to join the Tokoko Integrated Recovery Pathways Program in the Wide Bay region. Based in Bundaberg, the role involves evidence-based counselling, case management and recovery-focused support for people facing alcohol and other drug challenges.

The position offers a fixed-term appointment until 30 June 2027, 38 hours per week, SCHADS Level 5 salary plus superannuation and salary packaging benefits, with regular travel across

Qualifications

• Qualifications in Social Work, Counselling, Psychology, Community Services or related discipline, and/or relevant experience.
• Demonstrated experience delivering counselling and/or case management services.
• Knowledge of Alcohol and Other Drug treatment, recovery and harm minimisation principles.

Responsibilities

• Provide high-quality engagement, assessment, counselling and recovery-focused support services.
• Develop, implement and review individual recovery and treatment plans, including harm minimisation and psychoeducation.
• Deliver evidence-based counselling interventions.
• Support clients to identify and achieve their recovery goals.
• Provide case management and coordinated support services.
• Assist clients to access health, housing, wellbeing and community supports.
• Support treatment readiness, recovery planning and relapse prevention activities.
• Maintain continuity of care through referral, advocacy and stakeholder collaboration.
• Maintain accurate client records, case notes and reporting requirements.
• Participate in multidisciplinary case discussions and service planning activities.
