Job summary

Lives Lived Well in Bundaberg is seeking a passionate Program Coordinator to join the Tokoko Integrated Recovery Pathways Program. The role focuses on supporting clients transitioning from residential rehabilitation into sustainable community recovery, with emphasis on transition planning, housing pathways and ongoing recovery support.

The position is based in Bundaberg, involves travel across Bundaberg, Hervey Bay and the wider Wide Bay region, and is offered on a fixed-term basis to 30 June

Qualifications

• Experience within residential rehabilitation, transitional accommodation, homelessness services, aftercare or post-treatment recovery services will be highly regarded.

Responsibilities

• Support clients through pre-admission, transition and post-residential recovery pathways.
• Coordinate access to transitional accommodation and support successful tenancy outcomes.
• Assist clients to build independent living skills, recovery supports and community connections.
• Develop and implement transition and recovery plans tailored to individual needs.
• Support clients to access housing, health, employment, education and community services.
• Strengthen continuity of care through effective planning, communication and service coordination.
• Build and maintain strong referral pathways and relationships with community stakeholders.
• Support relapse prevention and ongoing recovery planning.
• Advocate for clients and assist them to navigate complex service systems.
• Maintain accurate documentation, case notes and reporting requirements.
