Job summary

Lives Lived Well in Bundaberg is seeking an experienced AOD Counsellor / Case Manager to provide therapeutic support, counselling and case management to people facing alcohol and other drug challenges.

You will work within a multidisciplinary team across the Wide Bay region, developing recovery plans, engaging clients through all stages of their journey, and helping them access health, housing and community supports while maintaining records.

Qualifications

• Qualifications in Social Work, Counselling, Psychology, Community Services or a related discipline, and/or relevant experience.
• Demonstrated experience delivering counselling and/or case management services.
• Knowledge of Alcohol and Other Drug treatment, recovery and harm minimisation principles.
• Strong recovery-oriented, trauma-informed and person-centred practice.

Responsibilities

• Provide high-quality engagement, assessment, counselling and recovery-focused support services.
• Develop, implement and review individual recovery and treatment plans, including harm minimisation and psychoeducation.
• Deliver evidence-based counselling interventions.
• Support clients to identify and achieve their recovery goals.
• Provide case management and coordinated support services.
• Assist clients to access health, housing, wellbeing and community supports.
• Support treatment readiness, recovery planning and relapse prevention activities.
• Maintain continuity of care through referral, advocacy and stakeholder collaboration.
• Maintain accurate client records, case notes and reporting requirements.
• Participate in multidisciplinary case discussions and service planning activities.
