are achievedLead project tracking, reporting and documentation activities, providing recommendations to support informed decision-makingBuild strong relationships with internal and external stakeholders to drive collaboration, engagement and successful project outcomesFacilitate workshops, meetings and continuous improvement initiatives to support organisational readiness and service excellenceMonitor compliance requirements, service agreements, policies and procedures to ensure alignment with aged care reforms and regulatory obligationsAnalyse service data and performance trends to identify opportunities for quality improvement, innovation and operational efficiencySupport the development of policies, guidance materials, tools and resources to strengthen service delivery and complianceManage project documentation, risk registers and reporting systems to ensure accurate and timely informationWhat You Will Bring Key Selection CriteriaTo be successful in this role, you will demonstrate:Tertiary qualifications in Project Management, Allied Health, Primary Care or a related discipline (desirable, but not essential)Proven experience building collaborative and productive relationships with diverse stakeholders to achieve shared outcomesExperience working within Aged Care services or supporting consumers navigating aged care systems and programsDemonstrated experience in project coordination, service improvement initiatives, quality improvement programs or organisational change projectsAbility to interpret legislative, funding or regulatory changes and develop policies, procedures and operational guidance materialsExperience in contract management, vendor management and client management/reporting systems such as Trak, Power BI, or the ability to quickly
