Job summary

healthAbility in Melbourne seeks a senior leader to guide our Care Management services, including Support at Home, and to drive day-to-day operations focused on person-centred care.

You will lead multidisciplinary teams, manage budgets, report on performance and risk, and partner with government and community stakeholders to deliver sustainable, high-quality health services across the eastern suburbs.

Qualifications

• Tertiary qualifications in Health, Community Services, Business Management, or related field.
• Extensive experience leading multidisciplinary teams and managing service delivery in aged care, community health, or similar regulated environments.
• Strong understanding of the Support at Home program, funding requirements, and quality standards.
• Demonstrated capability in financial management, budgeting, and forecasting.
• Experience leading change, implementing new programs, and driving service improvements.
• Sound understanding of quality, risk, compliance, and safeguarding requirements.
• Strong communication, problem-solving, and stakeholder engagement skills.
• Postgraduate qualifications or experience leading consumer-directed care programs would be highly desirable.

Responsibilities

• Lead the day-to-day operations of Care Management services, including Support at Home.
• Drive service improvements and reforms to keep services responsive and effective.
• Lead and support a team of Care Partners and senior staff, fostering a positive, collaborative culture.
• Monitor service performance, client outcomes, and key metrics for high-quality care.
• Manage program budgets, financial performance and revenue outcomes to support sustainability.
• Identify opportunities to improve systems, processes, and ways of working.
• Build strong relationships with clients, carers, providers, government agencies and other stakeholders.
• Provide reporting, insights and advice on performance, quality, risks and opportunities.
• Ensure services meet quality, safeguarding, compliance and risk management requirements.
• Collaborate with Executive - Primary and Complex Care to deliver organisational priorities.
