Better Health Network is seeking an experienced Program Manager to guide integrated community care and health promotion services, driving client-centred, culturally safe programs across Support at Home, Post Acute Care, and Community Health Nursing. Qualifications

• Experience leading health/aged care/community care or health promotion programs.
• Experience delivering client-centred and equity-focused health services.
• Strong budgeting, reporting and compliance skills.
• Ability to lead teams through change and drive service improvement.
• Proven ability to build partnerships with funders, government agencies and communities.

Responsibilities

• Lead day-to-day operations, service planning and workforce performance across community care and health promotion programs.
• Manage budgets, targets, quality indicators, reporting and funding compliance.
• Develop team capability through values-based leadership and succession planning.
• Strengthen referral pathways and partnerships with funders and stakeholders.
• Oversee health promotion planning, delivery, evaluation and reporting with focus on equity and priority populations.
• Drive service improvement and new service models informed by data and community needs.
