Job summary

Concentric is seeking a dedicated Physiotherapist for Homecare in Mornington, Victoria. This full-time role involves home-based assessments, developing care plans, and collaborating with clients, families and care teams to enhance independence and well-being.

You will document progress clearly, stay current with research, and participate in meetings and growth opportunities. Travel within Mornington and surrounding areas is required, with a supportive mentoring and education program available.

Qualifications

• Degree in Physiotherapy and current AHPRA registration.
• Willing to travel to Mornington and surrounding areas.
• NDIS Worker Screening Check and Working with Children Check.
• Car and valid Australian driver’s licence.

Responsibilities

• Assess clients in their homes to identify functional abilities and safety needs.
• Design plans to boost independence and quality of life.
• Collaborate with clients, families and colleagues for person-centred care.
• Document clearly and accurately current assessments and progress.
• Stay updated with the latest research and best practices.
• Engage in meetings, case chats, and professional development.
