Job summary

Concentric is seeking a qualified Occupational Therapist to join our Frankston-based team, delivering home assessments and person-centred therapy for clients in their own homes across Frankston, Mornington and surrounding areas.

You will design care plans to boost independence, collaborate with families and colleagues, document clearly, and stay abreast of current best practices while enjoying flexible hours and a supportive clinical mentoring program.

Qualifications

• Degree in Occupational Therapy and current AHPRA registration are required.
• Willing to travel to Frankston, Mornington and surrounding areas.
• NDIS Worker Screening Check and Working with Children Check are mandatory.
• Driver's licence and access to a car are required.

Responsibilities

• Conduct in-home assessments to identify functional abilities and home modification needs.
• Design plans and strategies to improve independence and quality of life.
• Collaborate with clients, families and colleagues for person-centred care.
• Maintain clear, accurate documentation of assessments and plans.
• Stay updated with latest research and best-practice guidelines.
• Participate in meetings and ongoing professional development.
