Job summary

Concentric AB in Melbourne, VIC seeks a casual Physiotherapist for the Aged Care stream. You will assess, plan and deliver person-centred physiotherapy across home and community settings with a multidisciplinary team.

You will provide education, maintain records and stay current with best practices while benefiting from a supportive mentoring program and flexible hours in a values-driven culture.

Qualifications

• Degree in Physiotherapy (or equivalent) with eligibility for registration.
• Current AHPRA registration and a valid Australian driver’s licence.
• NDIS Worker Screening Check and Working with Children Check current or willing to obtain.

Responsibilities

• Conduct comprehensive assessments to identify functional abilities, limitations, and goals.
• Develop individualised treatment plans to maximise independence, function and quality of life.
• Provide evidence-based therapeutic interventions.
• Collaborate with clients, families and a multidisciplinary team for person-centred care.
• Communicate effectively with stakeholders.
• Deliver individual and group therapy to enhance daily living, mobility and self-care.
• Educate clients, caregivers and support workers on physiotherapy interventions.
• Maintain accurate client records and documentation.
• Stay updated with current research, best practices and regulatory changes.
• Participate in team meetings and professional development to support a positive culture.

About your new role:

• Conduct comprehensive assessments of individuals to identify their functional abilities, limitations, and goals.
• Develop individualised treatment plans and intervention strategies to maximise independence, functional capacity, and quality of life.
• Provide evidence-based therapeutic interventions.
• Collaborate with clients, their families, and a multidisciplinary team to develop person-centred care plans and ensure holistic support.

• Deliver individual and group therapy sessions to enhance daily living skills, cognitive abilities, mobility, and self‑care.
• Provide education and support to clients, caregivers, and support workers to enhance their understanding of physiotherapy interventions.
• Maintain accurate and up-to-date client records and documentation.
• Stay informed about the latest research, best practices, and regulatory changes.
• Participate in team meetings, case conferences, and professional development activities and contribute to a positive workplace culture.
