NSW Health’s St Vincent’s Public Hospital in Sydney seeks a Physiotherapist for the Transitional Aged Care Program (TACP). This max-term full-time role runs until February 2027 with a Mon-Fri 8:30am-4:30pm schedule.

Qualifications

• A tertiary qualification in Physiotherapy is required.
• Current registration as a Physiotherapist with AHPRA is required.
• Driver Licence (NSW Class C or above) and ability to travel in the community.
• Ability to work autonomously and within a multidisciplinary team.
• Strong communication and interpersonal skills; commitment to professional development.

Responsibilities

• Deliver physiotherapy assessments and evidence-based interventions in the Transitional Aged Care Program.
• Provide case management to maximise client independence and rehabilitation goals.
• Collaborate with community services, healthcare professionals, clients and families to coordinate care.
• Contribute to quality improvement initiatives across the Transitional Aged Care Team.
• Provide education and support to colleagues within the team and wider services.
