Job summary

St Vincent's Private Hospital Toowoomba in Sydney CBD NSW invites applications for a Physiotherapist to join the Transitional Aged Care Program. This max-term, full-time role delivers assessments and evidence-based interventions to clients in the community, with case management to maximise independence and coordinated care with the MDT.

The successful candidate will hold a physiotherapy degree and current AHPRA registration, a NSW Driver Licence and be able to travel within the community,

Qualifications

• A tertiary qualification in Physiotherapy and current registration with AHPRA.
• NSW Driver Licence (Class C or above) and ability to travel within the community.
• Ability to work autonomously and in a multidisciplinary team.
• Excellent communication and interpersonal skills; commitment to professional development.

Responsibilities

• Deliver physiotherapy assessments and interventions for Transitional Aged Care Program clients in the community.
• Provide comprehensive case management to maximise independence and rehabilitation goals.
• Collaborate with community services and multidisciplinary teams to coordinate care.
• Contribute to quality improvement and education within the team.
