Bayside Health invites applications for a Grade 2 Physiotherapist within the Transition Care Program (TCP) in Rehabilitation, Ageing, Pain and Palliative Care Services. You will conduct high‑quality assessments and provide therapy to older adults transitioning from hospital to home, across diverse conditions, while collaborating with families and the multidisciplinary team.

Qualifications

• Bachelor of Physiotherapy and Masters of Physiotherapy Practice required.
• Current general registration with AHPRA is required.
• Minimum 2 years professional experience with older adults.
• Current Driver's Licence must be held.
• Demonstrated participation in Professional Development.

Responsibilities

• Deliver high‑quality assessment and therapy across a diverse aged care caseload (orthopaedic, neurological, respiratory, falls).
• Collaborate closely with patients, families and the broader healthcare team.
• Contribute to service improvement and support Allied Health Assistants and students.
