About the role
In this role, you will conduct physical, mobility, and fall-risk evaluations, deliver targeted exercise programs, strength training, pain management, source and prescribe relevant mobility aids and equipment in community-based settings under the guidance and support of a Senior Transition Care Program (TCP) Physiotherapist.
The role requires excellent communication, documentation, prioritisation and time-management skills, with the ability to work independently while contributing to a multidisciplinary, goal-focused reablement program.
The Transitional Aged Care Program (TACP) is a supportive multidisciplinary team made up of Clinical Care Coordinators and Allied Health Clinicians, including occupational therapy, physiotherapy, social work, speech pathology and dietetics.
