Job summary

Mercy Health Australia invites a casual care professional to join Mercy Place Yarrawonga Woods Point, Yarrawonga, VIC. You will provide quality patient-centered care, assess residents' wellbeing and support end-of-life care with dignity and respect within a multidisciplinary team.

Follow the aged care code of conduct, participate in continuous improvement, and bring a Certificate III or IV in Individual Support.

Qualifications

• Excellent clinical, time management, organisational and communication skills, with the ability to adapt and problem solve.
• A compassionate, flexible and positive approach to effective patient care and management.
• The ability to nurture friendly, respectful relationships and contribute to a collaborative team environment.
• Certificate III or IV in Individual Support.

Responsibilities

• Provide quality client-focused care to patients, their family and carers, using a holistic approach.
• Assessment and monitoring of patients' physical and emotional state.
• Support end-of-life care with dignity, comfort, and respect for personal wishes.
• Follow the aged care code of conduct.
• Participate in continuous improvement, including feedback, training, and adapting care practices.
• Document and communicate care information accurately within the organisation and with external providers.
• Be part of a multi-disciplinary team which requires you to provide highly responsive and flexible service to consumers and carers.
