• Follow the daily tasks and schedule to ensure a safe and comfortable living environment with physical and emotional support.
• Provide support using the Person-Centered Active Support Model to engage clients in their own lives.
• Assist with personal care, mobility, showering, continence, oral hygiene and meals.
• Administer medication and undertake other health care interventions.
• Promote a positive image of clients within the community.
• Maintain accurate client notes and ensure they are up to date.
• Report accidents, incidents and potential hazards to the Team Leader.
• Contribute to a culture of excellence and professional relationships with carers and stakeholders.
