Job summary

Achieve Australia is seeking a Disability Support Worker to provide person-centered care for clients with disability. You will help clients set goals, support daily living activities, and promote independence in the community.

The role emphasizes safety, respect, and collaboration with families, with checks like police and NDIS verifications part of the process. Training and supportive supervision are provided.

Qualifications

• Strong interpersonal, role modelling and customer service skills.
• Problem-solving abilities with adaptability to changing environments.
• Understanding of NDIS, Code of Conduct and Workplace Health and Safety.
• Awareness of human rights and challenges facing individuals with disabilities.

Responsibilities

• Provide an enjoyable, respectful and safe environment for all people we support.
• Deliver personal/physical care including grooming, toileting, meals, home support and medication administration.
• Assist with use of aids and devices and other personal equipment as required.
• Support participation in hobbies, relationships and community activities.
• Help clients access transport services including public transport and Achieve Australia vehicles.
• Advocate for clients and families to engage with community.
• Complete client documentation including case notes and medical records.
