Job summary

Pathway Talent is seeking compassionate Community Support Workers in Dubbo to join a respected community services organisation. This permanent part-time role offers rostered shifts and meaningful work helping people live independently and participate in their communities.

You will provide person-centred support based on each customer’s needs and support plan, including daily living activities, personal care and community access. A rewarding opportunity to make a difference.

Qualifications

• Experience in disability, mental health or community services.
• Person-centred and recovery-focused approach.
• Strong communication and relationship-building skills.
• Ability to work independently and respond calmly to changing situations.
• Good written communication and documentation skills.

Responsibilities

• Support customers with daily living activities and personal care.
• Assist with community access, appointments and social activities.
• Provide medication assistance according to procedures.
• Help customers develop independence and practical life skills.
• Assist with household tasks and meal preparation.
• Maintain accurate case notes and documentation.
• Follow individual support plans and positive behaviour support strategies.
• Promote customer choice, dignity, safety and wellbeing.
• Work collaboratively with families, carers and other professionals.
