Job summary

YWCA Canberra is seeking a Pathways Housing & Support Coordinator to provide culturally safe, person-centred case management and tenancy support. You will work with clients to access housing, healthcare, legal and migration services, education and employment pathways while navigating visa uncertainties.

The role involves collaboration with other agencies, quality improvement of the Housing Support Unit, and delivering trauma‑informed, strengths-based support to individuals and families.

Qualifications

• Diploma in Community Services or tertiary qualification in a related discipline.
• Experience delivering case management and advocacy for vulnerable groups.
• Understanding trauma-informed, strengths-based and culturally responsive practice.

Responsibilities

• Undertake comprehensive assessments and develop individualised support plans.
• Provide outreach and case management support to individuals, children, families.
• Assist clients to access housing, healthcare, migration, legal services, education and employment.
• Provide advocacy and coordinated referrals to support services.
• Facilitate case conferences and collaborative service planning.
• Maintain accurate case notes, records and reporting.
