Job summary

Uniting is seeking a Neighbourhood Care Partner to lead case management and deliver person-centred home and community care services. You will be the central contact for clients, coordinating care planning within available funding, and ensuring services support independence at home.

The role requires travel across the ACT and surrounding areas, and a commitment to safety, quality, and compliant practice in a not-for-profit setting.

Qualifications

• Experience in aged care or home and community care case management.
• Ability to manage client care budgets and plan services within funding.
• Experience using client management systems; valid driver’s license and access to a vehicle.
• First Aid certification and ability to coordinate with healthcare providers.

Responsibilities

• Lead case management and delivery of person-centred home and community care services.
• Facilitate delivery of quality services to help clients stay independent at home.
• Serve as central contact for clients and coordinate care planning within funding.
• Travel across ACT and surrounding areas to visit clients.
• Advocate for client choice and flexible services for independence.
