Job summary

IFYS invites applications for two Transition Case Managers to join the Fresh Start team in Maroochydore, Queensland. You will support youths aged 15–24 involved with Youth Justice or Community Corrections, helping them reach education, training and employment goals through individualized plans and brokerage management.

The role emphasizes risk assessment, stakeholder collaboration, and goal-driven support within a kaupapa of inclusivity and well-being for staff and clients alike.

Qualifications

• Professional experience working with young people in a therapeutic capacity.
• Excellent written and verbal communication skills, including report writing.
• Demonstrated knowledge of strengths-based and client-centred practice.
• Cultural knowledge of First Nations communities and culturally respectful engagement.

Responsibilities

• Receive and assess referrals, determining eligibility and priority of response.
• Support young people to identify needs and goals via individualized plans.
• Assist with job and training readiness and pathways to education, training and employment goals.
• Connect with local employers to identify work experience and employment opportunities.
• Assess risk levels and engage other supports as required.
• Complete administrative duties and records in line with requirements.
• Collaborate with Youth Justice and Community Corrections.
