Job summary

Anglicare Central Queensland is seeking a Homelessness Case Manager in Emerald to support people who are homeless or at risk of homelessness. You will develop and action case management plans, provide practical and personal support, risk assessments, and referrals to housing, health, and legal services.

This is a part-time fixed-term role offering a meaningful contribution to Central Queensland communities.

Qualifications

• Certificate IV qualifications in Community Services or similar.
• Ability to obtain a Blue Card (working with children) or exemption.
• Ability to obtain a Yellow Card (disability services) or exemption.

Responsibilities

• Work closely with participants to develop, action and review case management plans to meet their needs.
• Provide practical and personal support, crisis budgeting, information, advice, and referrals.
• Provide initial and ongoing assessment of participant needs, including risk assessments and safety planning as needed.
• Provide proactive, reliable, and respectful services to maximise independence.
• Advocate and assist participants to access housing and specialist services (e.g., drug/alcohol, mental health, legal, domestic and family violence).
