Job summary

Launch Housing is seeking a Case Manager in the Southbank area to support clients experiencing or at risk of homelessness. The role involves developing individualized case plans, linking clients to housing options and services, and coordinating multi-agency support to promote independence and dignity.

The position is a maximum-term, part-time role (68.4 hours per fortnight) based at Southbank, ending 30 June 2027.

Qualifications

• Relevant tertiary qualification in human services.
• Demonstrated ability to work with clients who have complex needs related to homelessness.
• Strong communication, interpersonal and teamwork skills.
• Excellent organizational and time management abilities.

Responsibilities

• Engage with clients to determine eligibility and refer to relevant services.
• Develop and monitor case plans with clients.
• Support clients to explore longer-term housing options.
• Maximize access to government and community income support.
• Maintain contact with vulnerable clients using flexible engagement strategies.
• Coordinate required services and monitor effectiveness.
