Admire Care is seeking a dedicated Disability Support Worker to join our Perth team on a part-time, on-site basis. You will provide in-home and community support to participants, assisting with daily living tasks such as personal care, meal preparation, household chores and medication prompts, while promoting independence within each participant's NDIS plan.

You will document progress and incidents accurately, and communicate regularly with participants, families and allied health professionals.

Qualifications

• Experience delivering person-centered disability support and daily living assistance.
• Strong communication and interpersonal skills with participants and families.
• Ability to document accurately and follow individual support plans.
• Understanding of privacy, confidentiality, trauma-informed, and rights-based practice.
• Current First Aid & CPR, NDIS Worker Screening Check and valid driver’s license.
