Job summary

Uniting NSW.ACT is seeking a Permanent Part Time Case Manager in Grafton NSW to lead case management and deliver person-centred home and community care services, helping clients live at home longer while staying connected to their local community.

You will be the central point of contact for clients, plan services within funding, travel between sites, and participate in governance, quality and safety activities while adhering to Uniting's values and policies.

Qualifications

• Certificate IV in Aged Care or Diploma of Community Service; Diploma or Bachelor in a relevant field highly regarded.
• Experience using client management systems.
• Must hold a current Australian driver’s licence, own vehicle and comprehensive car insurance.
• Hold a current First Aid Certificate.

Responsibilities

• Advocate for client choice, informed decision making and flexibility – ensuring the voice of the client is at the centre of service provision.
• Plan services for clients that meet their individual needs within funding.
• Identify and elevate vulnerable and at-risk clients and those with complex care planning needs.
• Participate in governance, continuous improvement and risk management activities to deliver quality and safe services.
• Ensure adherence to Uniting values, policies and procedures and relevant legislative requirements.
