Job summary

Uniting is seeking a Neighbourhood Care Partner to lead case management and deliver person-centred home and community care services. You will be the central contact for clients, coordinating care planning within available funding, and ensuring services support independence at home.

The role requires travel across the ACT and surrounding areas, and a commitment to safety, quality, and compliant practice in a not-for-profit setting.

Qualifications

  • Experience in aged care or home and community care case management.
  • Ability to manage client care budgets and plan services within funding.
  • Experience using client management systems; valid driver’s license and access to a vehicle.
  • First Aid certification and ability to coordinate with healthcare providers.

Responsibilities

What You’ll Be Doing

Responsible for leading case management and the delivery of person-centred home and community care services. In this role, you will facilitate the delivery of person-centred quality services to support our clients to live in their homes for longer and stay connected to their local community.

The role will be the central point of contact for clients and be responsible for care planning services to support individual needs based on available funding.

Key Role Responsibilities
  • Some travel required to visit clients within their homes across the ACT and surrounding areas.
  • Advocate for client choice, informed decision making and flexibility to support independence, social engagement, and quality of life.
  • Plan services for clients that meet their individual needs within their funding budget.
  • Identify and elevate vulnerable and at-risk clients, as well as those with complex care planning needs.
  • Participate in local governance, continuous improvement, and risk management activities to ensure that we are delivering quality and safe services.
  • Ensure adherence to Uniting’s values, policies, and procedures as well as relevant legislative requirements.
  • Ensure integration and collaboration across Home and Community Care, and Uniting.
  • Ensure the safety and wellbeing of yourself, clients, and others always and undertake work in a safe manner in accordance with Uniting policies, procedures, and directives.
  • Collaborating with team members to lead and coordinate of service delivery activities.
What We’re Looking For

Typically, this role requires prior experience within the aged care space including Home and Community Care within case management or strong understanding of Community services, as well as:

  • A strong understanding of the Support at Home program.
  • Previous experience communicating with clients, client families and the boarder healthcare community.
  • Have managed client care budgets.
  • Experience using client management systems.
  • Must hold a current Australian driver’s licence, own vehicle, and comprehensive car insurance.
  • Hold a current First Aid Certificate.
