Barwon Health’s Cancer Services Pharmacy team seeks an Oncology Informatics (CHARM) Lead Pharmacist to oversee safe prescribing via the CHARM system. This senior role sits in the Oncology Pharmacy leadership and shapes treatment pathways, formulary management and system improvements across Barwon Health and Colac Area Health.

The role emphasizes governance, medication safety and evidence-based practice through strong collaboration with multidisciplinary teams and service improvements.

Formación

• Undergraduate or postgraduate pharmacy qualification with current AHPRA registration.
• Hospital/clinical pharmacy experience with cancer services preferred.
• Experience with governance and medication safety in oncology.

Responsabilidades

• Develop and maintain CHARM treatment pathways and guidelines.
• Oversee CHARM drug formulary and compounding workflows.
• Coordinate user access, training and system support with IT/vendors.
• Lead governance, pathway audits and continuous improvement.
