Job summary

Barwon Health is seeking a senior CHARM Lead Pharmacist within Cancer Services Pharmacy to lead governance, development and safe prescribing through the CHARM electronic prescribing system. You will work closely with the Associate Director of Pharmacy, Cancer Services, and multidisciplinary teams to drive system improvements and safe cancer care.

Qualifications

Undergraduate or postgraduate pharmacy qualification, current AHPRA registration and at least eight years' hospital and clinical pharmacy experience, including a minimum of three years in cancer services.
Sound clinical knowledge of cancer treatments and a commitment to medication safety, evidence-based practice and quality improvement.
Strong analytical, organisational and problem-solving skills, with the ability to prioritise workloads, meet deadlines and lead practice improvements.
Excellent communication and stakeholder engagement skills, with the ability to collaborate across multidisciplinary teams and support system users.

Responsibilities

Develop, review and maintain CHARM treatment pathways, including standard-of-care regimens and clinical trial protocols, ensuring alignment with current evidence and governance requirements.
Oversee compounding worksheets and the regional CHARM drug formulary, ensuring accurate medicine information and compliance with relevant standards.
Coordinate user access, training and system support, liaising with software vendors and IT teams to troubleshoot issues and implement system upgrades.
Lead CHARM governance and continuous improvement, including pathway audits, medicines usage reporting, policies and procedures, and business continuity planning.
