Uniting is seeking an experienced Neighbourhood Care Partner to join our Home and Community Care team in Dubbo. You will manage client care plans, coordinate services, and advocate for client choice and dignity while ensuring funding is used appropriately.

You will travel between client homes and Uniting sites, building strong relationships with clients, families, and service providers, and contributing to quality improvement and safe work practices.

Qualifikationen

• Experience in aged care or Home and Community Care case management.
• Knowledge of the Support at Home program.
• Experience communicating with clients and families.
• Ability to manage client care budgets.
• Experience with client management systems.
• Must hold a valid Australian driver’s license, own vehicle, and comprehensive car insurance.
• Hold a current First Aid Certificate.
• Passion for supporting older people.

• Provide person-centred case management and develop individual care plans.
• Coordinate services and support clients to achieve goals and maintain independence.
• Advocate for client choice, dignity and informed decision-making.
• Manage client budgets and ensure services remain within available funding.
• Build relationships with clients, families, carers and service providers.
• Monitor and review care plans and client outcomes.
• Travel between client homes, Uniting sites and community locations.
