Job summary

Uniting in Dubbo, NSW, seeks a Neighbourhood Care Partner to provide case management and coordinate Home & Community Care services. You will support clients to live independently at home, manage budgets, and work with families and providers across Dubbo and nearby towns.

You will travel to client homes and Uniting sites, ensuring person-centred plans and timely reviews while upholding quality and safety standards. A genuine passion for helping older people is essential.

Qualifications

• Experience in aged care/Home and Community Care settings, focusing on case management or community services.
• Ability to manage client budgets and funding within available resources.
• Experience using client management systems and writing clear client records.

Responsibilities

• Provide person-centred case management and develop individual care plans.
• Coordinate services and support clients to achieve goals and maintain independence.
• Advocate for client choice, dignity and informed decision-making.
• Manage client budgets and ensure services stay within funding.
• Build strong relationships with clients, families, carers and service providers.
• Monitor and review care plans, client outcomes and ongoing support needs.
• Identify and escalate complex or high-risk situations.
• Maintain accurate client records and documentation.
• Work collaboratively with Team Leaders and other internal teams.
• Respond to client feedback and complaints appropriately.
• Contribute to quality improvement, risk management and safe work practices.
• Travel between client homes, Uniting sites and community locations as required.
