Job summary

Uniting NSW & ACT is seeking a Neighbourhood Care Partner for our Home & Community Care team in Dubbo, NSW. The role involves case management, care planning and coordinating person-centred services to help clients live independently at home.

You will be the main contact for clients and networks, tailoring services to funding and preferences, and traveling to client homes and Uniting sites as required.

Qualifications

• Experience in aged care, especially Home and Community Care.
• Ability to manage client care budgets and allocate funds.
• Strong communication with clients, families and health professionals.
• Must hold a current Australian driver’s licence, own vehicle and comprehensive car insurance.
• Hold a current First Aid Certificate.

Responsibilities

Uniting is seeking an experienced Neighbourhood Care Partner to join our Home and Community Care team. You will be responsible for case management, care planning and coordinating person-centred services that support clients to remain living in their own homes for longer.

As a key point of contact for clients and their support networks, you will ensure services are tailored to individual needs, preferences and available funding.

About the role

You will:

• Provide person-centred case management and develop individual care plans.
• Coordinate services and support clients to achieve their goals and maintain independence.
• Advocate for client choice, dignity and informed decision-making.
• Manage client budgets and ensure services remain within available funding.
• Build strong relationships with clients, families, carers and service providers.
• Monitor and review care plans, client outcomes and ongoing support needs.
• Identify and upscale complex or high-risk situations.
• Maintain accurate client records and documentation.
• Work collaboratively with Team Leaders, Care Excellence Leads and other internal teams.
• Respond to client feedback and complaints appropriately.
• Contribute to quality improvement, risk management and safe work practices.
• Travel between client homes, Uniting sites and community locations as required.

About you

You will have:

Typically, this role requires prior experience within the aged care space including Home and Community Care within case management or strong understanding of Community services, as well as:

• A strong understanding of the Support at Home program.
• Previous experience communicating with clients, client families and the boarder healthcare community.
• Have managed client care budgets.
• Experience using client management systems.
• Must hold a current Australian driver’s licence, own vehicle, and comprehensive car insurance.
• Hold a current First Aid Certificate.
• A genuine passion for supporting older people and making a positive difference.
