Job summary

Uniting is seeking an experienced Neighbourhood Care Partner to join our Home and Community Care team. You will be responsible for case management, care planning and coordinating person-centred services that support clients to remain living in their own homes for longer.

You will work with clients and their networks to tailor services to individual needs and funding, travelling across Dubbo and surrounding regions as required.

Qualifications

• Experience in aged care/Home and Community Care with case management.
• Experience communicating with clients, families and the broader healthcare community.
• Experience managing client care budgets.
• Experience using client management systems.
• Must hold a current Australian driver’s licence, own vehicle and comprehensive car insurance.
• Hold a current First Aid Certificate.
• A genuine passion for supporting older people.

Responsibilities

• Provide person-centred case management and develop individual care plans.
• Coordinate services and support clients to achieve goals and maintain independence.
• Advocate for client choice, dignity and informed decision-making.
• Manage client budgets and ensure services stay within funding.
• Build relationships with clients, families, carers and service providers.
• Monitor and review care plans and client outcomes.
• Travel between client homes and Uniting sites as required.
