Job summary

Uniting is seeking an experienced Neighbourhood Care Partner to join our Home and Community Care team in Dubbo, NSW. You will manage client care plans, coordinate services and support independent living for older people in their own homes.

You will work closely with clients, families and service providers, travel between client homes and Uniting sites, and ensure care is person-centred and within funding constraints. A driver’s licence and First Aid are required.

Qualifications

• Prior experience in aged care, including Home and Community Care, in case management.
• Ability to develop and monitor client care plans.
• Experience managing client budgets and funding.
• Driver’s licence, vehicle and comprehensive car insurance.
• Current First Aid certificate.
• Commitment to person-centred care and community involvement.

Responsibilities

• Provide person-centred case management and care plans.
• Coordinate services and support clients to stay independent.
• Advocate for client choice, dignity and informed decisions.
• Manage client budgets and funding.
• Maintain accurate client records and documentation.
• Travel between client homes and Uniting sites.
• Collaborate with Team Leaders and other internal teams.
• Respond to client feedback and raise issues as needed.
