Job summary

OnSide Support Coordination is hiring a dedicated Support Coordinator in Australia to assist clients with their NDIS plans. You will connect clients to community resources, coordinate providers, and support goal achievement through plan-based services.

This permanent role offers flexible work patterns, including home office days and client visits, across multiple office locations. You will work with a national team to deliver person-centered, accessible supports.

Qualifications

• Ability to build relationships and coordinate supports across providers.
• Strong organisational and time-management skills.
• Eligibility to work in Australia and compliance with NDIS checks.
• Completion of the NDIS Worker Orientation training before starting.

Responsibilities

• Support OnSide clients to build capacity and understand their NDIS plan.
• Coordinate supports across multiple providers for each client.
• Plan coordination to enable greater independence across life domains.
• Work within funding levels and monitor budgets for each client.
• Gather and document information for evidence-based reports and updates.
• Collaborate with stakeholders to achieve client NDIS goals.
