Job summary

OnSide is seeking a dedicated Support Coordinator to provide coordination and transition supports for clients under NDIS plans. You will work across children, youth and adults to gather resources needed to achieve goals, with a mix of home, community visits and office days.

The role offers flexible work arrangements—1–2 days from home, 1–2 days visiting clients, and 1 day in the office—full-time or 0.8 FTE minimum, with strong focus on client outcomes and collaboration within a national service

Responsibilities

• Coordinate supports to suit clients across multiple providers.
• Assist clients to navigate their NDIS plan and goals.
• Track funding budgets and document updates for reports.
• Liaise with stakeholders to coordinate service delivery and linkages.
