Job summary

Tradewind Australia partners with a values-driven organisation to place an experienced NDIS Support Coordinator in a permanent, full-time position in Melbourne. The role is based at the Scoresby office with a hybrid working arrangement and involves managing a caseload of participants with complex needs.

You will coordinate and implement NDIS plans, liaise with NDIA and service providers, and advocate for participant goals while documenting progress and maintaining quality records.

Qualifications

• Previous experience in an NDIS Support Coordination role.
• Strong understanding of the NDIS framework and plan implementation.
• Experience supporting participants with ABI, MS, Parkinson's or Dementia highly regarded.
• Ability to independently manage a caseload and work autonomously.
• Excellent communication, organisation and stakeholder engagement skills.

Responsibilities

• Coordinate and implement NDIS plans in line with participant goals.
• Manage a caseload and coordinate appropriate supports and services.
• Build relationships with participants, families and external stakeholders.
• Liaise with the NDIA, allied health professionals and service providers.
• Monitor participant progress, advocate for their needs and maintain accurate documentation.
