Job summary

Tradewind Australia Pty is seeking an experienced NDIS Support Coordinator (Level 2) to join a values-driven team in Melbourne. The role is permanent, full-time, with hybrid working from the Scoresby office, coordinating supports for participants with complex needs.

You will liaise with NDIA and service providers, advocate for participant goals, and maintain accurate records while building strong relationships with families and stakeholders.

Qualifications

• Experience in an NDIS Support Coordination role is desirable.
• Strong understanding of the NDIS framework and plan implementation.
• Ability to independently manage a caseload and work autonomously.
• Excellent communication and stakeholder engagement skills.

About the Organisation

About the Role

Based at the Scoresby office with a hybrid working arrangement, you will manage a caseload of NDIS participants with complex needs.

Key Responsibilities

• Coordinate and implement NDIS plans in line with participant goals.
• Manage a caseload and coordinate appropriate supports and services.
• Build relationships with participants, families and external stakeholders.
• Liaise with the NDIA, allied health professionals and service providers.
• Monitor participant progress, advocate for their needs and maintain accurate documentation.

About You

• Qualification in Community Services, Disability, Social Work, Psychology or related field desirable.
• Previous experience in an NDIS Support Coordination role.
• Strong understanding of the NDIS framework and plan implementation.
• Experience supporting participants with ABI, MS, Parkinson's or Dementia highly regarded.
• Ability to independently manage a caseload and work autonomously.
• Excellent communication, organisation and stakeholder engagement skills.
