Job summary

WellVibe is a Melbourne-based provider delivering disability and community support services, seeking an experienced NDIS Support Coordinator (Level 2) to manage a caseload with hybrid working. The role focuses on plan implementation, funding management, and stakeholder coordination to achieve participant goals.

You'll work independently, maintain high standards of compliance and communication, and travel across Melbourne as required.

Qualifications

• Minimum 3 years' direct NDIS Support Coordination experience.
• Ability to independently manage an allocated caseload.
• Current NDIS Worker Screening Clearance.
• Valid Victorian driver's licence and capacity to travel across Melbourne.

Responsibilities

• NDIS Plan Implementation & Funding Management: interpret plans, funding categories and budgets.
• Coordinate disability, allied health, community, housing and mainstream services.
• Monitor plan utilisation and funding sustainability throughout the plan period.
• Maintain comprehensive case notes and relevant documentation.
• Build and maintain professional relationships with participants and stakeholders.
