Job summary

Yarra Valley Support Coordination in Lilydale, VIC is seeking an experienced Support Coordinator to manage a busy caseload of NDIS participants. You will work with participants, families, providers and professionals to implement plans and connect supports, while maintaining accurate case notes and documentation.

You will be a confident communicator with strong time-management skills, able to navigate complex situations, and thrive in a collaborative, supportive team.

Qualifications

• NDIS Support Coordination experience or related role.
• Strong understanding of NDIS participant plans.
• Excellent written and verbal communication skills.

Responsibilities

• Build professional relationships with participants and support networks.
• Help participants understand and implement their NDIS plans.
• Connect participants with providers and community supports.
• Coordinate services and maintain communication between stakeholders.
• Monitor plan utilisation, budgets and service delivery.
• Identify gaps, risks or barriers in support arrangements.
• Support plan reassessments and changes in circumstances.
