Job summary

Ableconnect Pty Ltd invites applications for an NDIS Support Coordinator based at Doncaster East, Victoria. This is a full-time, 6-month fixed-term contract with an hourly rate of $42–$45 plus superannuation.

You will coordinate supports, connect participants with providers and help participants leverage their NDIS plans. You will manage a caseload, build strong networks across the NDIS and community services, advocate for participants and maintain accurate records.

Qualifications

• Minimum 3 years experience as NDIS Support Coordinator.
• Strong knowledge of NDIS and participant journey.
• Established networks within NDIS, disability and community services.
• Proactive in building professional relationships and developing referrals.
• Confidence engaging with providers and stakeholders.
• Excellent written documentation and case-note skills.
• Ability to independently manage a caseload.

Responsibilities

• Coordinate supports and services in line with participants' goals and NDIS plans.
• Manage an allocated caseload of NDIS participants.
• Build relationships with participants, families, providers and stakeholders.
• Develop and maintain networks with support providers and allied health professionals.
• Engage with existing networks and develop new connections in the NDIS community.
• Identify gaps in supports and connect participants to our services.
• Assist participants to understand and navigate the NDIS.
• Maintain accurate case notes and documentation.
• Escalate risks or concerns and advocate for participants.
