Job summary

SustainHealth is a Sydney-based, multi-award-winning healthcare recruitment consultancy seeking an experienced NDIS SIL Intake & Growth Manager to lead the SIL program growth in South Western Sydney.

Reporting to the Director, you will own the referral pipeline, build partnerships with coordinators and hospital discharge teams, and guide moves from enquiry to move-in while ensuring safety and sustainability.

Qualifications

• 3+ years in NDIS SIL intake, business development or similar roles.
• Proven track record of SIL referrals you’ve sourced and converted into move-ins in the last 12–24 months.
• Strong relationships with referrers across Sydney.
• Solid knowledge of SIL funding, roster ratios, tenancy separation and service agreements.
• Understanding of the NDIS Code of Conduct, conflict of interest, consent and participant choice.
• Confidence using a CRM and reporting clearly.
• Cultural understanding and connection with South Western Sydney’s diverse communities; Arabic is highly regarded.
• Experience with hospital discharge, psychosocial disability, complex health, behaviours of concern, guardianship or justice interfaces.
• NDIS Worker Screening Check (or eligibility to obtain one), Working with Children Check, NSW driver license and own vehicle.

Responsibilities

• Build and grow referral channels with coordinators, hospital discharge teams, allied health and advocates.
• Own the pipeline in the CRM from enquiry through funding checks, assessment, visits and move-in.
• Match people to vacancies where needs and funding can be supported safely.
• Lead transitions with operations, clinical and housing teams for smooth handovers.
• Report weekly to leadership on referrals, conversion and forecast move-ins.
