Victorian Home and Community Care Pty Ltd (VHCC) is seeking a Community Worker to assess participant needs, translate NDIS plans into practical supports, and help participants work toward their goals across greater Melbourne.

Based at Box Hill, you will manage plans within budget, coordinate linked services, document progress, and report to the NDIS and relevant agencies, ensuring person-centred, rights-based care.

Qualificações

• Excellent communication and interpersonal skills.
• Strong organisational and time-management abilities.
• Ability to maintain accurate records and documentation.
• Experience in community services or disability support preferred.
• Knowledge of NDIS processes and compliance requirements.

Responsabilidades

• Interview participants and families to assess needs.
• Prepare and implement individualised NDIS plans.
• Advise participants on the NDIS process and rights.
• Support participants to achieve plan goals and monitor progress.
• Review plans and goals with participants and families.
• Coordinate supports to build independent living capacity.
• Connect participants with funded supports and community services.
• Maintain relationships with NDIS and service providers.
